JASNA Event Questionnaire

In the past year, our attendance at the May and December events has been less than anticipated. I have created a questionnaire to encourage feedback about these events. Do not worry about offending me. Just be honest and let me know what kind of events you would like to attend. Many thanks for your participation!

If you did not attend the May and December events in 2012, please let me know why. (Was it the day of the week, the time of day, a prior commitment, the cost, the food, or the program?) *

What kind of programs would you like to see at these events? *

What would you be willing to pay to attend an event? Also, would you rather pay a $10 cover charge and purchase your own meal or not have food served? *

Rather than a lunch event, would you prefer a brunch or tea? (Note: A tea is very expensive because of the preparation time involved.) *

Where would you like these events to be held? (All ideas are welcome!) On which day of the week would you like the events to be held? *
